How to Prepare for Your Visit to the Clinic

Make an appointment.

If we do not pick up the phone, we are probably with a patient. Please leave a message and we will call you back the same day, sometimes within the hour.
Think about all of your medical needs.

Prepare to share them with your healthcare provider.

During your visit, we will address all your concerns.
Make a list of your medications, including prescribed, over-the-counter, and supplements.

Include the dosage information, including the amount and the frequency.

If you need a refill of your medications, bring the original bottle or box with the prescriber's information.
